Discharge Planning
Planning for your discharge starts at the time of admission and includes identifying whether you may have problems coping at home following discharge. Some patients may require rehabilitation at a community hospital when the acute phase of their illness is over.
Discharge summary
Each patient will be given a GP letter and a letter for their practice nurse who deals with Coronary Heart Disease.
Outpatient appointment
If you require an outpatient's appointment this will usually be sent to you in the post but may be arranged prior to discharge.
Medication
If you need to take medication following your discharge you will usually receive a seven days supply.
Transport
Booking hospital transport is limited and reserved ONLY for those who have no other means. Therefore, we ask you to make your own arrangements wherever possible.
