Young Adults with Learning Disabilities

Young Adults with Learning Disabilities

The Learning Disability team are happy to support the transition of young adults aged 17-18 years. They may be able to come to outpatient appointments (in paediatric, adult or joint clinics); show young people the adult outpatient clinics or inpatient wards; and introduce them to less familiar parts of the hospital.

They also have a Transition Resource pack which is available to download, or as a hard copy from the learning disability office or in Children's Outpatients. Please contact the team to see how they can help, or find them in the Safeguarding Office near Children's Outpatients (B11).
