The Conversation Project

The Conversation Project was developed in the RUH to support conversations health care professionals have with patients, family and carers about their care in recognition of the potential for future deterioration. The Conversation Project supports Advance Care Planning (ACP).

The Conversation Project promotes conversations between healthcare professionals, patients and their families at this time of uncertainty. It focuses on the need to document clearly these conversations that have taken place and the decisions made and ensure that this information is transferred when the patient is discharged. In this way it facilitates Advance Care Planning.

The Principles of the Conversation Project are:
  • Earlier recognition of end of life or recovery uncertain
  • Supporting conversations about uncertain future and advance care planning for these patients and their families
  • Improving documentation of conversations related to end of life care to inform management plans
  • Improve sharing of information related to advance care planning on any transfer of care
