Requestors should alert the biochemistry laboratory when a request is being made (on extension 4240 Monday-Friday 9.00-17.00, via switchboard outside of these times)

For non-Millennium and non-ICE requests, please use a yellow antibiotics form and ensure that ALL details on the request form are completed.

Clinical Application
For once daily tobramycin therapy, pre-dose levels should be <1mg/L.
For bd or tds Tobramycin, when treating a gram negative pneumonia, pre-dose levels should be <2mg/L and post dose levels >7mg/L.

For bd or tds Tobramycin, when treating endocarditis, pre-dose levels should be <1mg/L and post dose levels >3-5mg/L.

Please note that it is the responsibility of the prescriber to check antibiotic levels and adjust dose/timing as necessary. Please discuss with a microbiologist or pharmacist if advice is required.
