Clinical Application
Careful monitoring of lithium is essential (including calcium, renal and thyroid function). NICE CG185 recommends measuring lithium every 3 months for the first year and every 6 months thereafter (or every 3 months for patients in certain groups).

NICE guidance is to try to maintain lithium concentrations below 0.8 mmol/L to reduce the risk of renal damage.

Typical treatment levels are often between 0.6 mmol/L – 0.8mmol/L, however this varies between patients and 0.4mmol/L may be sufficient. Higher levels can be needed during a relapse. Please refer to patient-specific guidelines from specialist team as appropriate.

If 12hr sample is 1.0 mmol/L or above please discuss with specialist team.

Please note: Phenylpropanol-amine and pseudoephedrine may cause falsely elevated results while Mefenorex may cause falsely low results.
