Occupational Health Clearance
To complete your Health Assessment you will need to return two documents to the Trust:-

Disclosure and Barring Service Check
Whether your require a Disclosure and Barring Service Check will depend on the post you have applied for.
If your post requires you to have a DBS check then you will need to contact the HR Department to arrange an appointment to complete the DBS form. You can arrange this by either responding to your conditional offer letter through the NHS Jobs system or by contacting ruh-tr.HR@nhs.net or 01225 825831.
Once you have completed your DBS form you will have completed the preliminary aspect of this pre-employment check. However, unless your check has been returned by the DBS prior to your start date you will be asked to complete a 'Declaration' form - which confirms that you do not have any undisclosed convictions or cautions – at Induction.
Failure to declare a conviction or caution may result in either the withdrawal of your offer of employment or the termination of your contract of employment with the Trust, so it is important that you declare any convictions or cautions to us if you did not do so during the application and interview processes.
If your post is based in Paediatrics or will allow you to have access to Children then we will need to have received your completed check from the Disclosure and Barring Service before you can start with us.
When visiting the HR department, please ensure that you have the following documentation with you when completing your DBS check:
- Three forms of valid, current and original identification, at least one of which should be photographic, for example:
- a current passport
- photocard driving licence (with counterpart)
- Biometric Residence Permit (UK)
- EU national identity card - EU only

IT Access
To access the Trust's IT systems you will need to be provided with a 'Smartcard'.A 'Smartcard' is a card the size of a credit card with a 'chip' provides you with secure (password protected) access to the Trust's systems.
If you do not already have a Smartcard, then the HR Team will need to generate one for you prior to you coming into post. You will need to make an appointment to attend the HR department to complete this process.

Starting Salary
Please be aware that all starting salaries will be offered at the base point of the salary scale unless you provide evidence of relevant experience which would affect this, such as an NHS payslip. If you have experience which you expect to affect your starting salary then please ensure that you provide a copy of this to the Recruitment Team.

aged 3 months to 5 years.
